Welcome to Your Online Patient Admission
At Bundaberg Private Day Hospital, we want your experience with us to be exceptional, straightforward and stress-free. We ask that you complete our Online Patient Admission Form at least 5 days before your admission, as this will:
- Provide our clinical staff with important information about your medical history so we can care for you safely and make sure you meet the admission criteria
- Enable us to process your paperwork so your admission is stress-free and seamless
- Allow enough time to cross-reference your private health fund insurance
Please click the link below to proceed with completing this important document. Some of this information you may have already provided to your Doctor, however, you will need to resupply this to our hospital for our records.
If you cannot complete the Patient Admission Form online, please visit our hospital at 51 Commercial Street, Kensington and a paper copy can be provided. You can complete your forms at the hospital and hand them in to our friendly administration team.
Please note: Your booking is not confirmed until we have received and processed this Admission paperwork.
Things to know when completing the Online Patient Admission:
- Allow 15-20 minutes to complete the form
- Although compatible with a mobile/ tablet device, we recommend completing this form on your computer
- New patients – fill in your name, email address and nominate a password to create your account
- Existing patients, please sign in using your previous login details
- Once logged in, click ‘Start a New Admission’ at the top of the page
- Provide as much detail as possible and answer all *mandatory fields
- Any items not related to your specific procedure, please leave these blank or indicate not applicable
Please have the following information ready:
- Personal details, including your weight, height, emergency contact details and Next of Kin
- GP and referral information
- Medicare, DVA or Pension card
- Private Health Insurance details
- Medical and surgical history
- List of all medications including non-prescription medicines, weight loss medication (if applicable), vitamins and natural supplements. Please disclose any side effects or allergic reactions to past medications.
- WorkCover details (if applicable)

- Completed hospital forms, consent form (if applicable) and any letters or reports from your doctor
- Health Insurance membership details
- Medicare card
- A list of any current medications
Where applicable, please also bring your:
- Pension card
- Healthcare card
- Prescription record card
- Pharmaceutical entitlement card
- Safety net card
- Department of Veterans’ Affairs card
- Workers’ compensation approval letter
- X-ray or ultrasound films that may be relevant
- Medications in original containers
It’s also a good idea to bring something to do while you wait – a book or magazine, headphones or an iPad. And make sure you wear comfortable clothes and shoes with non-slip soles and low heels.
We also recommend bringing a cardigan or something warm as the temperature inside the hospital can be cool.
Keeping your Personal Information Safe
Patients can have peace of mind knowing their personal data is stored in a secure, restricted SQL database, with all data encrypted at rest and in transit. We also engage in regular penetration testing to ensure our cybersecurity measures are reliable, and allow us to continuously refine and maintain a secure system.
