Credentialling
If you are interested in operating at Bundaberg Private Day Hospital, you are welcome to contact Kate or Dheena on the details below for a tour of the facilities and discuss potential operating sessions.
We will send you an online application form which will require you to upload:
- Your current CV
- A copy of your current AHPRA registration
- Two referees
- A copy of your current Professional Indemnity Insurance certificate of currency showing:
- Period of insurance
- Amount of cover
- Category of insurance (please note we do not accept copies of tax invoices, renewal notices or receipt).
The Medical Advisory Committee (MAC) reviews all applications once we receive the above information.
